How to register
If you live in our practice area and would like to register with us, please collect the relevant registration forms that are available at reception. It is important that you bring in your medical card or NHS number without these we cannot register you. You can register at both sites and you may be asked to attend at either Manby or North Somercotes surgery. On registration you can express a preference of Doctor; however, you will be registered with the practice rather than an individual GP. Upon returning forms to the surgery for registration we require proof of photo Identification, either passport or driving licence.
Where patients are requesting to join the practice list, the practice does not discriminate on the grounds of:
- Race, gender, social class, religion, sexual orientation or appearance.
- Disability or medical condition.
You can register as a temporary resident with Marsh Medical Practice if you are in the catchment area of the practice for more than 24 hours, but NO LONGER THAN 3 MONTHS.If you are going to be resident in the practice area for more than 3 months YOU MUST REGISTER AS A PERMANENT PATIENT.
If you try to temporarily register with the practice again within the same year, your registration status with the practice will become PERMANENT.
To Register as a temporary patient you will need:
Photographic identification i.e Passport/Driving licence, confirmation of home address.
A repeat medication slip or ask your doctor to fax your repeat medications to us on 01507 358746.
The practice offers a range of appointments that include mornings Monday to Friday and afternoon/evenings Monday to Friday. Our surgeries start at 8.30 am each morning and routine appointments are available throughout the day, the latest being 5.50 pm on a Friday. Surgeries alternate between the two sites at North Somercotes and Manby, with all the Doctors offering routine appointments at both sites Monday to Friday. We also provide urgent/on the day appointments, where by ringing first thing in the morning, 8am, you may be able to get an appointment to see a GP that same day. The Doctor on call for that day may be working at either site, we do our best to offer an appointment at the site closest to your home, but in an emergency you may be asked to attend the alternate site.
The surgery at Manby closes at 1.00 pm on Tuesdays and Thursdays
The practice has a system for receiving and returning telephone calls from patients. Patients are asked to telephone to speak to a Doctor after 10.30 am. Should the Doctor not be available at this time, the receptionist will take the patient details to enable the Doctor to call them back on the same day. This information is entered on to the relevant Doctor’s appointment page so that a reminder is given and clarification is available that the call has been made.
Patients who want to speak to a Nurse on the telephone will be transferred to the Nurse if available at that time. If the Nurse is not available to take the call immediately, the receptionist will take all patient details to enable the Nurse to call them back on the same day. This information is entered on to the relevant Nurse page so that a reminder is given and clarification is available that the call has been made.
At Marsh Medical Practice, we aim to treat all our patients promptly, courteously and in complete confidence.
We ask that in light of not being able to attend an appointment, to please contact the surgery as soon as possible as that appointment can then be offered to someone else.
The practice policy for requesting a home visit is that firstly the patient’s details will be taken by the receptionist and passed on to the on call Doctor. The on call Doctor will ring the patient and make an assessment over the telephone. At this stage the Doctor may ask the patient to attend the surgery if possible, or the Doctor will visit the patient at home. At no point will a request for a home visit be refused by the practice.
Any requests for home visits should, whenever possible, be made before 10.30 am.
Clinics and services
|Ante-natal care||The community Midwife undertakes antenatal care, by appointment on a Tuesday morning. The appointment may be offered at Louth Hospital.|
|Post-natal care, gynaecology, cervical smears||Examinations are done by appointment with a Doctor with a Practice Nurse in attendance. Our Practice Nurses can also carry out cervical smears by appointment.|
|Minor surgery||Minor surgery may be undertaken at the Practice by arrangement with the Doctor carrying out the procedure. This will usually be carried out in the treatment room with the Practice Nurse in attendance. For medico legal reasons patients are asked to sign a consent form before the procedure is carried out.|
|Child surveillance/vaccination and Immunisation||
Routine child health checks and immunisation are available by appointment.
For medico legal reasons a parent or guardian of the child is asked to sign a consent form before the vaccination is carried out.
All vaccines are stored in accordance with manufacturer’s instructions. Vaccine batch numbers being recorded for all vaccines administered.
Staff involved in administering vaccines are trained in the recognition of anaphylaxis and able to administer appropriate first-line treatment when it occurs.
The Health Visitor holds a baby clinic at both sites on a Tuesday – please telephone 01507 617790 to make an appointment.
|Family planning||Advice is available from our Doctors or the Practice Nurses. The Practice Nurse carries out routine annual checks for oral contraception. A patient needing a first prescription for contraception (including emergency contraception) will need to see a Doctor or the Nurse Practitioner.|
|Health promotion||We encourage all our patients to share the responsibility for their health, both in preventing disease and in treating existing diseases. Prevention really is better than cure. The risk of many serious diseases can be reduced by healthy life-style and without drugs.|
|Asthma clinic||Patients with asthma are also seen on a regular basis by appointment with the Practice Nurse. Please ask at reception for information.|
|Coronary heart disease clinic||Our Practice Nurses run a clinic for monitoring and advising patients who have suffered a stroke or who have coronary heart disease. Please ask at reception for details.|
|Smoking cessation clinic||
If patients are smokers, they should be advised to stop smoking. Their daily tobacco consumption will be added to their computer record. Smokers will be given written information in the form of an appropriate leaflet.

Ways in which you can order your repeat medication :-
- In person, at either Manby or North Somercotes surgery, just hand in the repeat slip that is given to you when collecting your medication.

Dosette Boxes (blister packs)
If you are having difficulty remembering to take your tablets, or are taking a large amount of tablets, why not try dosette boxes which allow you to maintain a correct daily medication regime. Please ask one of our dispensing team for details.
You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness.
Since our telephones are very busy in the early morning, we ask you to telephone for test results (eg blood, urine, x-ray) after 10.30am each day.
All results are seen before filed onto a patients notes, by the Doctors, if there are any problems, the Doctor will contact the patient directly.
To ensure confidentiality, we only release results to the patient, unless alternative arrangements have been agreed in writing.
The Practice also offers the following services that are not part of the NHS Service. A fee is normally charged for these services in line with the current recommendations of the British Medical Association.

Forms may be left at the surgeries for completion; however, the doctors are unable to complete them during surgery. Please make arrangements to collect them at a later date.

Please make an appointment with one of our Practice Nurses at least 10 weeks in advance of your holiday to ensure adequate cover. Following your consultation in the travel clinic, the Practice Nurse will give you a form which indicates what vaccination and/or anti-malarial tablets you will be given, also stating the amount you will have to pay. The cost associated with this, is listed below and you will be required to make payment to the practice before you are given the injection or receive the tablets. You can pay either cash, cheque or credit/debit card at reception/dispensary, who will also take the form and give you a reciept of payment confirmation.
